This authorization signifies that the institution meets state standards as outlined in the California Education Code (CEC) and Title 5 of the California Code of Regulations (CCR). However, the institution must not suggest that this approval indicates endorsement of its programs by the Bureau or that it surpasses minimum state standards.

Procedures for Admissions

            The Best Healthcare Institute, Inc. campus is located at 26236 Industrial Blvd, Hayward, CA 94545. The campus is a single-story building of approximately 380 square feet, with a front entrance. It includes a reception area/administrative office, a nursing skills lab, and a library/computer lab/resource center. Restrooms are conveniently available on-site.

            The Certified Nurse Assistant Training Program offers synchronous online instruction for the Theory portion, while clinical training is conducted off-site at contracted clinical facilities. Although instruction is delivered via an online Learning Management System, students are welcome to visit the campus for their personal learning needs. The institution adheres to all federal, state, and local regulations, including those related to fire safety, building safety, handicap access, and health. The facilities are well-lit, air-conditioned, and wheelchair accessible. Ample free parking, including handicapped spaces, is available at the front and side of the building.

            Since the course is conducted online, the school provides the necessary software, while students are responsible for their hardware, such as a computer with a microphone and headset. Supplies provided by the school and owned by the school are included in the tuition. The facility and equipment meet all federal, state, and local regulations, ensuring compliance with safety and health standards.

Admissions Policies and Procedures

Procedures for Admissions

          Prospective applicants should visit Best Healthcare Institute, Inc. and meet with the RN Program Director. During the visit, the RN Program Director will offer a campus tour, provide detailed information about the institution’s programs and policies, discuss the applicant’s qualifications, and help identify the best path to meet their career goals. The office staff will also briefly cover tuition and available payment plans. If an inquiry is made by phone, the office staff will provide basic program information and invite the applicant to visit the campus. The application and enrollment process begins with an initial advising meeting. All applicants must have a personal interview with the RN Program Director and complete a tour of the facility before being accepted into a training program. The interview typically lasts around 15 minutes. During this time, the office staff will review the training programs, tuition, and entrance requirements. Qualified applicants can complete the application forms and enrollment agreement, or return at a later date to enroll.
Applicants are encouraged to observe classes if they wish.

          During the admissions interview, applicants are assessed for their potential to secure employment in their chosen field. Evaluation factors include test scores, job history, background, attitude, professional appearance, and communication skills. A strong desire to succeed and a willingness to make sacrifices are crucial for completing the career program successfully. A positive attitude is essential for maximizing the learning experience and preparing for a new career. Additionally, students need to allocate time for attending classes and completing assignments, with an expectation of approximately one hour of preparation for each class session attended. Investing more time in preparation enhances the opportunity for success.

Admissions Criteria and Standards for the Certified Nurse Assistant Training Program:

            The Certified Nurse Assistant Training Program at Best Healthcare Institute, Inc. has specific admission requirements that applicants must meet.

To be considered for admission, a prospective student must:

1. Be at least 16 years old by the start of the program.
2. Attend a new student orientation.
3. Complete, sign, and submit the Best Healthcare Institute, Inc. Enrollment form.
4. Sign documents acknowledging receipt of disclosure forms as required by the California Private Postsecondary Education Act of 2009, Articles 11.
5. Provide proof of good health, including a medical history, physical examination, and PPD test or chest x-ray.
6. Be free of convictions or have clearance from the CA Department of Public Health and DOJ.
7. Demonstrate the ability to exert 20-50 lbs. of force occasionally, 10-25 lbs. of force frequently,
and/or up to 10 lbs. of force constantly to move objects.
8. Have student liability insurance.
9. Possess a TIN or SSN to qualify for the certification exam.
10. Own a laptop/computer, microphone, headset, and have access to the Internet.

Definition of Contact Hours, Transferability of Credits and Granting of Transfer Credit

        Contact hours refer to the duration of classes during which students receive direct
instruction from a faculty member. Each contact hour requires a full 60 minutes of instruction within a 60-minute period.

NOTICE CONCERNING TRANSFERABILITY OF CREDIT AND CREDENTIALS EARNED AT OUR INSTITUTION

          The transferability of credits earned at Best Healthcare Institute, Inc. is entirely at the discretion of the institution to which you may transfer. Similarly, acceptance of any degree, diploma, or certificate you earn here is also subject to the policies of the receiving institution. If the credits or credentials you obtain at Best Healthcare Institute, Inc. are not accepted by the institution you wish to transfer to, you may be required to retake some or all of your coursework there. Therefore, it is important to ensure that your attendance at this institution aligns with your educational goals. This may involve contacting the institution you plan to transfer to after attending Best Healthcare Institute, Inc. to verify if your credits or credentials will be accepted. Best Healthcare Institute, Inc. does not have any articulation or transfer agreements with other institutions and does not accept transfer credits or experiential credits from other institutions, nor credits earned through challenge exams or ability-to-benefit requirements.

Attendance

✓Students are expected to attend all classes regularly and arrive on time for both theory and clinical assignments. Theory classes are conducted synchronously online, while clinical sessions take place in person at a clinical facility.

 ✓Instructors are responsible for recording student attendance using the Institute’s online attendance tool.

✓If a student cannot attend a theory class or clinical session, they must notify the instructor at least one hour before the start of the class or clinical assignment.

 ✓Upon returning to online classes, students must promptly arrange with the instructor to complete any make-up work.

✓A student will be dismissed from the program if they exceed two absences (excused or unexcused) in total, including one absence in theory and one in clinical. Dismissal is determined by the Director if the student accumulates more absences than allowed.

Retention of Records

            The Registrar at Best Healthcare Institute, Inc. is responsible for maintaining official student and financial records in a fire-proof file cabinet within a secure storage room on-site, and for ensuring these records are accurately recorded in the institution’s software system. Additionally, the Registrar generates rosters, grade sheets, and transcripts, and maintains attendance records.

            In accordance with CCR 71930, Best Healthcare Institute, Inc. will retain student records for five years from the last day of attendance, completion, or withdrawal, as mandated by state law. Student transcripts will be kept indefinitely.

Students may inspect and review their records upon written request (using a petition form). An appointment can be scheduled through the Registrar’s Office for an on-site review or via video conferencing with a representative from the institution. Records stored in the cloud and system are password-protected, with students needing a 4-digit PIN for access.

             Students have the right to access their records during the institution’s normal business hours. They may either view their records in the school office during these hours or schedule a time that is convenient for both the student and the school administration. If an appointment is  requested, it will be scheduled within 48 hours. Academic records and transcripts from Best Healthcare Institute, Inc.’s Certified Nurse Assistant Training Program are maintained electronically, with hard copies also kept in the student’s file.

Student Grievance Procedure

          A grievance is defined as a complaint regarding any alleged unauthorized or unjust action or decision by a student, faculty member, administrator, or staff that adversely affects a student’s status, rights, or privileges. To ensure openness, accountability, and responsiveness, the institution has established grievance procedures.

          The institute maintains a file for each reported grievance, documenting the procedures followed and the final resolution of the case. If a student wishes to formally submit a grievance, they must provide a written complaint to Best Healthcare Institute, Inc. This written grievance should clearly include the student’s name, a detailed description of the complaint, the names of all parties involved, and any relevant documentary evidence.

          If students have exhausted these procedures and the problems have not been resolved to their satisfaction, they have the right to contact the Bureau of Private Postsecondary Education by calling toll free 1 (888) 370-7589 or by completing a complaint form, which can be obtained on the bureau’s Internet Web site www.bppe.ca.gov. Course Description:

            Prepares the Certified Nurse Assistant Student for the State of California Nursing Assistant Competency Evaluation. Certification is after satisfactory completion of the course program and passing of the CA State Exam. This course introduces the students to the role of a Certified Nurse Assistant with instruction in basic skills and competencies. Theory classes will be held in the online classroom and clinical hours will be at the contracted training facilities or laboratory. Additional 8 hours for the skills lab at the clinical site.

Course Admission Requirements:

✓ Must be 16 years old.

✓ Provide proof of good health, including a medical history, physical examination, and PPD
test or chest x-ray. This report, signed by a healthcare provider, must confirm that the
student does not have any health conditions that could pose a risk to themselves, patients,
or visitors. This documentation is required before starting clinical training.

✓ Must be free from any criminal convictions (excluding minor traffic violations) or have
clearance from the California Department of Health Services. Submit a completed Live Scan
form before beginning clinical training.

✓ Must be able to exert 20-50 lbs. of force occasionally, 10-25 lbs. of force frequently, and/or
up to 10 lbs. of force constantly to move objects.

Course Objectives:

Upon completion of the 160-hour course, the Certified Nurse Assistant will be able to:

Graduation Requirements:

To receive a Certificate upon graduation, a student must meet the following requirements:

These are included in the theory hours. Discussion, selected readings and group/individual
conferences are conducted outside of theory hours.

In-Person: Clinical Experiences

Attendance/Make-up Policy:

        Students are expected to attend all class sessions and clinical hours. If an unavoidable absence occurs, the instructor must be notified in advance. It is mandatory for students to make up any missed theory or clinical classes. Missed theory classes must be completed before attending clinical hours.

          Best Healthcare Institute, Inc. allows up to two (2) absences (either excused or unexcused) during the program—one for theory and one for clinical. These absences must be made up before the program is completed. Students who exceed two absences, whether excused or unexcused, will be dropped or withdrawn from the program.

Make-Up Work:

Make-up work is required for any absence. Students must make every effort to attend classes and clinical training regularly. The institution provides two (2) make-up sessions—one for theory and one for clinical—during the program. Make-up classes will cover the same content and participation requirements as the originally scheduled classes. Any missed online instruction due to program-related technological issues will be made available for make-up without penalty.

Leave of Absence Policy

      At Best Healthcare Institute, Inc., we recognize that unforeseen circumstances may
necessitate a temporary interruption in a student’s studies. To support these situations, we offer a Leave of Absence (LOA) policy.

    Students can request a leave of absence for reasons such as medical issues (including
maternity leave), bereavement, military service, and jury duty. However, LOAs cannot be used to avoid dismissal due to unsatisfactory academic performance or failure to meet attendance requirements. An LOA is generally limited to 60 calendar days, and only one LOA is permitted within any 12-month period.

To request an LOA, students must follow these procedures:

 1. Contact the RN Program Director to obtain an LOA Request Form.
2. Submit the request in writing before the intended start date of the LOA.
3. Provide any necessary documentation to support the request.

In exceptional cases where prior approval is not feasible (e.g., hospitalization due to an
accident), retroactive approval for the LOA may be considered.

  The RN Program Director will review and decide on LOA requests. If a student does not
return on the scheduled date following their LOA, they will be withdrawn from the program.

Grade and Evaluation Policy:

Theory Evaluation

Students will be assessed based on the following criteria:

  • Quizzes        70%
  • Final Exam  20%
  • Attendance 10%

Note: Instructors reserve the right to adjust the grading breakdown.

    At Best Healthcare Institute, Inc., we are committed to providing prompt evaluations and feedback. We guarantee a turnaround time of no more than 72 hours from receipt of student assignments, quizzes, and completed checklists, to the delivery of our evaluation or response. Your progress is important to us, and we are dedicated to offering timely feedback throughout your course.

Clinical Evaluation

  Students will receive a list of nursing skills to master, which includes both performance
techniques and related theoretical knowledge.

The grading scale for the clinical portion is as follows:

SATISFACTORY – student demonstrates ability to complete clinical objectives safely, accurately, and in a timely manner with minimal supervision.
NEEDS IMPROVEMENT – student needs close supervision and/or additional time to complete assignments.
UNSATISFACTORY/FAILED – student does not demonstrate improvement nor meet clinical objectives.

Satisfactory Academic Progress (SAP)/Probation/Dismissal:

    Best Healthcare Institute, Inc.’s Satisfactory Academic Progress (SAP) standards measure qualitative (cumulative grade point average) progress toward the completion of the student’s program of study. The SAP standards are applied to all students and represent a minimum standard of academic achievement required by the institution for continued enrollment.

SAP Evaluation Periods

     The institute’s SAP standards measure a student’s satisfactory academic progress during the program. An academic grade report will include the student’s grades earned in Theory and Clinicals. The end-of-the- program grade report will provide cumulative information for all modules attempted and completed and a cumulative grade point average

    Satisfactorily completed course hours include those for which a student receives a grade other than a “W” or “F.” All courses for which a student receives a grade, whether passing or failing, a withdrawal (W), a repeated course, or an incomplete (I), are counted in determining credits attempted. Students who do not meet the SAP Minimum Standards will receive written notification stating that the student is being placed on an Academic Warning. A student on Academic Warning who brings their CGPA and completion rate up to SAP standards during the program is removed from Academic Warning.

Satisfactory Academic Progress Appeals, Probation, and Academic Plans

      When a student is withdrawn because he/she failed to make SAP, he/she may appeal that result. A student who is not meeting the SAP standards following Academic warning will be withdrawn from the institution. He or she may appeal this action to the RN Program Director based upon extenuating circumstances. These might include the death of a relative, an illness of or injury to the student or other extraordinary situations. The appeal must contain 1) an explanation of why the student failed to meet the SAP standards; and 2) a description of what has changed in the student’s situation that will enable him or her to again meet the satisfactory progress standards. Supporting documentation should be submitted if possible. The RN Program Director will review the information submitted in the context of the student’s entire academic record, and notify the student of his or her decision within 24 hours.

    Students who have been terminated from the institution for failure to achieve satisfactory academic progress may petition for readmission. The RN Program Director may grant re-admission of a terminated student upon receipt of the student’s petition. The re-admitted student is placed on academic probation. If the student fails to increase his/her GPA to 2.00 or more, he/she will be dismissed. Plans for correction/improvement may either be a practice and demonstration of the skills needing improvement in the skills laboratory or tutoring/retesting. The institute is only allowed to administer one reexamination to the student. A follow-up conference shall then be scheduled and noted on the probation form for the student’s reevaluation. Successful remediation shall be documented accordingly. If the outcome of the remediation is unsuccessful, the student may repeat the program. Failure to pass the same certification program the second time will result in termination or dismissal from the school. An official letter of termination or dismissal shall be sent to the student. The student will have 7 business days to accomplish and pass the remediation.

Classroom Rules of Conduct

Students are expected to:

● Demonstrate the expected good classroom conduct of a Certified Nurse Assistant student.
● Demonstrate professionalism by showing honesty in class participation.
● Observe school policy regarding school uniforms.
● Observe proper conduct provided by the instructor during class hours. (Example: turning off cell phones, pagers and computers.)

Student Requirements:

** Best Healthcare Institute, Inc. reserves the right to change or cancel a class when the minimum number of students is not met or due to unavoidable circumstances with sufficient notice.

Student Activities and Services

Academic Advising – Students are advised on both personal and academic issues and are encouraged to discuss their scholastic and vocational goals. The faculty and RN Program Director has a sincere interest in the personal welfare of each student and therefore an open door policy is employed.

Research and Library Services – Best Healthcare Institute, Inc. will provide an online library that is accessible to all students. The online library will integrate EBSCO host online services to form a virtual library capable of conducting research. A student may search and find textbooks, periodicals, news and current events, encyclopedias and dictionaries, economic data, and business and health directories. A student may conduct a search by title, author, subject area, or key words.

Professional Counseling Referrals – Students requesting professional counseling services will be referred to local counseling agencies around the bay area.

Placement Services – Best Healthcare Institute, Inc. will provide job assistance by providing employment workshops and employment referrals through Best Healthcare Providers, a home health agency. The school will also assist with resume preparation, job leads, and job interviews with other employers may be set up for the graduate. Although the school does provide job placement assistance, it cannot guarantee placement or level of income or wages. Students are encouraged to attend recruiter presentations when they become available. Applicants will be vetted and hiring is based on the standard qualifications of the employer. There is no guarantee of employment.

Housing Services – Best Healthcare Institute, Inc. does not offer or coordinate housing. Listings of nearby apartment complexes are available as a resource aid. Best Healthcare Institute, Inc. assumes no responsibility to find or assist a student in finding housing. Students and their parents are responsible for housing arrangements.

Student Tuition Recovery Fund

    The State of California established the Student Tuition Recovery Fund (STRF) to relieve or mitigate economic loss suffered by a student in an educational program at a qualifying institution, who is or was a California resident while enrolled, or was enrolled in a residency program, if the student enrolled in the institution, prepaid tuition, and suffered an economic loss. Unless relieved of the obligation to do so, you must pay the state-imposed assessment for the STRF, or it must be paid on your behalf, if you are a student in an educational program, who is a California resident, or are enrolled in a residency program, and prepay all or part of your tuition.

    You are not eligible for protection from the STRF and you are not required to pay the STRF assessment, if you are not a California resident, or are not enrolled in a residency program.

     It is important that you keep copies of your enrollment agreement, financial aid
documents, receipts, or any other information that documents the amount paid to the school. Questions regarding the STRF may be directed to the Bureau for Private Postsecondary Education, 1747 North Market Blvd., Suite 225, Sacramento, California, 95834, (916) 574-8900 or (888) 370-7589.

To be eligible for STRF, you must be a California resident or enrolled in a residency program, prepaid tuition, paid or deemed to have paid the STRF assessment, and suffered an economic loss as a result of any of the following:

1. The institution, a location of the institution, or an educational program offered by the institution was closed or discontinued, and you did not choose to participate in a teach-out plan approved by the Bureau or did not complete a chosen teach-out plan approved by the Bureau.

2. You were enrolled at an institution or a location of the institution within the 120 day period before the closure of the institution or location of the institution, or were enrolled in an educational program within the 120 day period before the program was discontinued.

3. You were enrolled at an institution or a location of the institution more than 120 days before the closure of the institution or location of the institution, in an educational program offered by the institution as to which the Bureau determined there was a significant decline in the quality or value of the program more than 120 days before closure.

4. The institution has been ordered to pay a refund by the Bureau but has failed to do so.

5.The institution has failed to pay or reimburse loan proceeds under a federal student loan program as required by law, or has failed to pay or reimburse proceeds received by the institution in excess of tuition and other costs.

6. You have been awarded restitution, a refund, or other monetary award by an arbitrator or court, based on a violation of this chapter by an institution or representative of an institution, but have been unable to collect the award from the institution.

7. You sought legal counsel that resulted in the cancellation of one or more of your student loans and have an invoice for services rendered and evidence of the cancellation of the student loan or loans.

To qualify for STRF reimbursement, the application must be received within four (4) years from the date of the action or event that made the student eligible for recovery from STRF.

       A student whose loan is revived by a loan holder or debt collector after a period of noncollection may, at any time, file a written application for recovery from STRF for the debt that would have otherwise been eligible for recovery. If it has been more than four (4) years since the action or event that made the student eligible, the student must have filed a written application for recovery within the original four (4) year period, unless the period has been extended by another act of law.

      However, no claim can be paid to any student without a social security number or a taxpayer identification number.

Per BPPE: Effective April 1, 2024, the Student Tuition Recovery Fund (STRF) assessment rate will change from two dollars and fifty cents ($2.50) per one thousand dollars ($1,000) of institutional charges to zero dollar ($0.00) per one thousand dollars ($1,000) of institutional charges.

Student’s Waiver of Rights Void

            The student’s participation in the grievance procedure and the disposition of a student’s grievance shall not limit or waive any of the student’s rights or remedies. Any document signed by the student that purports to limit or waive the student’s rights and remedies is void. The RN Program Director will be responsible for maintaining and keeping the log book of student grievances.

Enrollment Dates

        Instruction is hourly based; Students will be advised at start dates at the time they
enroll. Students will be notified in advance concerning important scheduling information such as operating hours, holidays, class schedules and revisions to the schedule. The following legal holidays will be observed:

– New Year’s Day        – Memorial Day         – Independence Day

– Labor Day                 – Thanksgiving           – Christmas Day

        Appointments and walk-ins are welcome; however, it is strongly recommended that graduates who wish to have job placement assistance make an appointment to meet with a representative.

Cancellation, Withdrawal, and Refund Policy

Student’s Right to Cancel

1. Right to Cancel: You can cancel your agreement for a program of instruction without any penalty or obligations through attendance at the first-class session or the seventh calendar day after enrollment, whichever is later. After this cancellation period, you also have the right to stop attending school at any time and receive a pro rata refund if you have completed 60 percent or less of the scheduled days in the current payment period of your program through the last day of attendance.

2. How to Cancel: Cancellation occurs when you provide written notice of cancellation to Best Healthcare Institute, Inc., 26236 Industrial Blvd, Hayward, CA 94545. This can be done by mail or hand delivery to the HR Manager or Registrar.

3. Effective Date: If sent by mail, the written notice of cancellation is effective when properly addressed with proper postage and deposited in the mail.

4. Form of Cancellation: The written notice of cancellation does not need to follow any particular form and is effective if it shows that the student no longer wishes to be bound by the enrollment agreement.

5. Refund Process: If the enrollment agreement is canceled, the school will refund any money paid by the student, less the application/registration fee, within 45 days after the notice of cancellation is received, if applicable.

6. Withdrawal: A withdrawal can be effectuated by the student’s written notice or by the student’s conduct, including, but not limited to, a student’s lack of attendance.

Withdrawal from the Program

Pro Rata Refund: You may withdraw from the school at any time after the cancellation
period and receive a pro rata refund if you have completed 60% or less of the scheduled
days in the current payment period of your program through the last day of attendance.
The refund will be less the application/registration fee and will be processed within 45 days
of withdrawal. If you have completed more than 60% of the period of attendance for which
you were charged, the tuition is considered earned, and you will not receive a refund.

Deemed Withdrawal: For refund determination, you shall be deemed to have withdrawn
from a program of instruction when any of the following occurs:

-You notify the institution of your withdrawal, or as of the date of your withdrawal, whichever is later.

– The institution terminates your enrollment for failure to maintain satisfactory progress,
failure to abide by the institution’s rules and regulations, excessive absences, and/or failure to
meet financial obligations to the school.

-You fail to attend class for 2 days.

Library and Other Learning Resources

  Best Healthcare Institute, Inc. has established a comprehensive Learning Management System available at https://besthealthcareinstitute.com/courses/nurse-assistant-training
program/ with the Provider Landing Page URL at https://besthealthcareinstitute.com/.

     Online learning for the Theory portion will be done via zoom or google meet. Other
resources will be made available via an electronic library and resource center consisting of electronic texts and periodicals providing information in the nursing discipline offered by the institution. The electronic resource center provides students with access to current publications with a number of URLs for free online libraries and research organizations.

      Best Healthcare Institute, Inc. will provide an online library that is accessible to all
students. The online library integrates EBSCO host Electronic Journals Service (EJS) as the student’s gateway to thousands of e-journals containing articles from hundreds of publishers. A student may search and find textbooks, periodicals, news and current events, and dictionaries. A student may conduct a search by title, author, subject area, or key words. The institution will provide EBSCO log-in information to the student (https://www.ebsco.com/).

Placement Services

    The school will provide job placement services, which include resume preparation, job leads, and job interviews with employers may be set up for you. Although the school does provide Job placement assistance, it cannot guarantee placement or level of income or wages. Students are encouraged to attend recruiter presentations when they become available. A Performance Fact sheet will report all Gainful employment data for Certified Nurse Assistant under Occupational Code: 31-1014.00

Loans

  If the student obtains a loan to pay for an educational program, the student will have the full responsibility to repay the full amount plus any interest due, less the amount of any refund. If the student receives federal student financial aid funds, the student is entitled to a refund of the money not paid from federal financial aid funds. If the student is eligible for a loan guaranteed by the federal or state government and the student defaults on the loan, both of the following may occur: The federal or state government may take action against the student, including applying any income tax refund to which the student is entitled to reduce the balance of the loan. The student may not be eligible for any other federal student financial assistance or other government assistance until the loan is repaid.
